Custom Physical Therapy is an outpatient physical therapy practice treating a broad range of physical conditions. Our number one priority is you, the patient. We provide personalized treatment with hands-on, compassionate care. We do not rush our patients or our clinicians. Rather we perform comprehensive evaluations and encourage your input for treatment planning and goal setting.

Andy Lodato, PT

Andy started with Custom PT in August of 2020. He came in to take over for Andre’ and build on Custom’s foundation. Andy will mainly work out of the Sparks location. “Once I met Andre’ and the Custom team, I knew this was a special place.”

Andy is originally from Albuquerque, NM but, practiced in the Seattle area for 20 years. During that time, he co-owned a multi-site clinic for 12 years. Andy relocated with his family to Reno in 2019 and has not looked back. Drawn by the great weather, outdoor opportunities Reno is a fabulous place to call home!

Andy received his Masters degree from UTEP and went back later in his career to get his Doctorate from Texas Tech Health Sciences Center. His main practice focus is orthopedics with patients aging from 7-99 years of age.

Andy has two kids: Daughter Riley and son Matthew. Outside of work, I am usually chasing kids around. If not doing that, then I am playing pickleball, golfing, or fly fishing.

Monica Hill Physical Therapist

Monica Hill, PT, DPT

Monica Hill is a Physical Therapist originally from San Diego where she traded in a surfboard for skies when she moved to Reno. She has been treating for 3.5 years in the Reno area. She graduated from Northern Arizona University undergraduate and then proceeded to get her DPT from the University of St Augustine of San Marcos. Monica completed BSPTS Schroth-Rigo level 1 scoliosis training 1.5 years ago where she gets to help improve the posture and function of people with scoliosis. Monica loves treating all orthopedics to help people return to their favorite activities.

Joshua Perez, PT, DPT, Cert. DN

Josh started with Custom Physical Therapy in 2020 and is the director of the NW Reno clinic.  “I came to Custom PT because I wanted to be a part of a company that puts the patient first.  Also, I love NW Reno”

Josh is from Auburn, CA.  He received his BS in Kinesiology at San Jose State University.  He went to the University of St. Augustine in Austin, TX, and received his Doctor of Physical Therapy, graduating with high honors.  Coming out of PT school, he took a job near Savannah, Georgia, and became a Clinic Director out there.  When the opportunity to come back home and become a clinic director at Custom PT arose, he jumped on it.

Josh enjoys treating all orthopedic conditions.  His goal is to diagnose the tissue causing the problem, educate you on the best research evidence to treat it, and empower you with the movements that will promote tissue healing.

Allison Yarborough, PT, DPT, PRPC (She/her)

Allison has her bachelor’s degree in nutrition and dietetics from Texas Christian University and her Doctorate in Physical Therapy from Rocky Mountain University of Health Professions. She has completed specialized education in the treatment of the pelvic floor for all genders through Herman and Wallace, and received her Pelvic Rehab Provider Certification from them in 2021.
Allison was born and raised in Reno. She most recently moved from Salt Lake City, and has also lived in Fort Worth, TX, Los Angeles, California, and Brussels, Belgium, and enjoys traveling to new and exciting places. In her free time, Allison enjoys hiking, backpacking, running, and spending time with her family and friends in the beautiful outdoors.

Yes. The choice of where you receive therapy is always yours. While the majority of our patients come to us by physician referral/recommendation, many clients request to see us due to our strong clinical reputation in the community.

We operate in a direct access state where, if your insurance plan authorizes self-referral to physical therapy, a physician referral is not necessary for you to be seen by a licensed therapist. Please check with your insurance though as many health insurance plans require a physicians referral for therapy to be covered.We also offer treatments, services and/or products that do NOT require a referral and are available on a self-pay basis.

We participate in the majority of major health insurance plans including Medicare and Worker’s Compensation. Please contact us prior to your first visit to see if we participate with your specific health plan.

As a service to our patients, we will gladly file claims with your insurance company on your behalf. In order to appropriately submit these claims, we will need to obtain a copy of your insurance identification card to ensure that we have the necessary information.

Your therapist will conduct an evaluation of your condition lasting between 60-90 minutes. The evaluation is necessary to create a program designed specifically to address your needs for a maximum recovery.

– Referral slip from your medical doctor
– Your medical insurance card
– Your ID
– If you are getting therapy as a result of a worker’s compensation claim or an automobile accident, you will also need to bring your claim information, including claim number.

Please arrive 15 minutes early on your first visit to fill out any necessary paperwork.

We recommend comfortable clothing such as sweatpants or shorts, a T-shirt or tank-top and sneakers.

Each visit will last 60 – 90 minutes on average. Times vary due to the treatment and exercises being performed.

The regularity of your visits will be determined after your initial evaluation, but most patients have 3 visits each week scheduled.
